What is sperm freezing (sperm cryopreservation)?

Sperm freezing, also known as sperm cryopreservation, is a process that involves collecting, analyzing, and preserving sperm cells at extremely low temperatures for future use in fertility treatments.

The sperm sample is stored in liquid nitrogen at −196°C. At this temperature, all cellular activity stops completely — preserving the sperm in its current state until it is thawed and used. The length of time a sample has been frozen does not affect its viability.

Who should consider sperm freezing?

Sperm production (spermatogenesis) continues throughout adult life, which means there is no strict age deadline for freezing. That said, sperm quality, motility, and volume do gradually decline and sperm fragmentation increases from around age 40 onward — so freezing a sperm sample earlier generally preserves a higher-quality sample.

Patients choose to freeze sperm for a wide range of reasons:

• Medical treatments: chemotherapy, radiation therapy, and testicular surgery can all affect sperm production. We recommend preserving sperm before starting any of these treatments. Testosterone replacement therapy should also prompt sperm freezing beforehand, as it suppresses natural sperm production.
• Gender-affirming care: for transgender women and non-binary individuals who produce sperm and are beginning or planning hormone therapy or gender-affirming surgery, freezing sperm beforehand preserves the option of biological parenthood in the future.
• Vasectomy: freezing a sperm sample before a vasectomy provides a backup if circumstances change.
• Occupational or lifestyle risks: military personnel, firefighters, contact sport athletes, and those regularly exposed to toxins, radiation, or extreme heat may choose to freeze sperm as a precaution.
• Personal or family planning: freezing sperm at a younger age preserves a higher-quality sample for use when you’re ready to start or grow your family.

The sperm freezing process: what to expect

The sperm process is quick, straightforward, and non-invasive:

1. Initial consultation and screening: a brief health evaluation that includes blood tests to screen for infectious diseases — a standard FDA requirement.
2. Collection and semen analysis: a semen sample is collected through masturbation in a private collection room at our clinic. We ask that you abstain from ejaculation for 2 to 5 days before your appointment. Our laboratory team will perform a semen analysis immediately to review sperm count, motility, and morphology before the sample is frozen.
3. Freezing: the semen sample is divided into several vials, mixed with a cryoprotective agent to protect the cells during freezing, and stored in liquid nitrogen.
4. Storage: your frozen sperm is stored at a long term storage facility until you are ready to use it. We’ll discuss storage terms and any storage fees at your consultation.

Using frozen sperm for fertility treatment: effectiveness and success rates

When you’re ready to use your stored sperm, our team will thaw the sample and recommend the most appropriate fertility treatment based on your situation. Options may include:

• Intrauterine Insemination (IUI): prepared sperm is placed directly into the uterus around the time of ovulation.
• Intratubal Insemination (ITI): prepared sperm is placed directly into the entrance of the fallopian tube, closer to where fertilization naturally occurs.
• In Vitro Fertilization (IVF): eggs are retrieved and fertilized with the thawed sperm in our laboratory, with the resulting embryo transferred into the uterus.

The effectiveness of sperm freezing is well established – success rates when using frozen sperm are comparable to those achieved with fresh sperm, provided the original sample was of good quality. The length of time your sperm has been stored does not affect its viability or the chances of a successful cycle.
