How the At-Home Sperm Test Works
- Order your at-home semen analysis kit. Complete the form on this page for a link to start your order. Your sperm sample test arrives in 3 business days.
- Abstain for 48 hours–5 days before producing your sample, per standard fertility testing guidelines.
- Produce your sample on a Monday, Tuesday, Wednesday, or Thursday morning, and register it using the QR code inside your kit. (Registration is required — without it, Fellow cannot release your results.)
- Drop it off at any UPS Store the same day. A prepaid shipping label is included, and Fellow’s preservation solution and temperature-controlled packaging keep your sample stable in transit.
- Get your semen analysis results in 2 business days. You’ll receive an easy-to-understand report via a secure link.

What happens next depends on why you tested:
Referred by an outside provider? If your medical provider asked you to complete a semen analysis before an official referral to The Fertility Center of Las Vegas, be sure to enter their name and fax number on the form. By completing this form, you authorize The Fertility Center of Las Vegas/Ovation Fertility to automatically fax a copy of your results to the provider and fax number entered.
